INTAKE CALL
15 MINUTES

We’ll chat and get to know each other. I’ll gather some basic information and outline a proposed care plan. Additionally, we’ll review insurance coverage, cost, and value.
INITIAL APPOINTMENT
1 HOUR
$100 - $200
We’ll take a deep dive and collaboratively establish initial interventions and goals. To optimize our time together, I’ll request that you complete some paperwork to ensure I have the necessary information to set you on a path towards success.
FOLLOW-UP APPOINTMENT(S)
15-30 MINUTES
$40 - $80
Small changes over time equate to big results. I’m here to help you make progress, track success, and determine if adjustments to your care plan are necessary. Follow-up plans will be individualized based on medical necessity and mutually agreed upon schedules.
